1. Completed Bio data, Letter of Statement, as well as stamped Form of Bio data for Degree Certificate and Academic Transcript
2. Proposal for cum laude if he/she meets the cum laude requirements (GPA > 3.70 and study period < 2 years)
3. Legalized copy of bachelor (S1) degree certificate
4. Copy of residential identity card and birth certificate
5. Evidences of free from tuition debt
6. 2 black and white passport photos of 4 x 6 and 2 black and white passport photos of 3 x 4.
7. Copy of thesis (including doc. and PDF files) and thesis presentation files (in ppt) in 1 (one) CD
8. Evidences of graduation fee payment / letter of graduation fee exemption
9. Evidences of thesis registration
10. Official report of the thesis defense signed by the Chairman of Defense Commission

1. Hard copy and URL of accredited international or national journals publishing the graduates’ scientific paper
2. The academic certificate should be taken no more than 1 year after the certificate is legally signed (D Day + 365 calendar day)
